Waiver Case Manager – Full Time
Job Responsibilities:
• Assessing the areas of the client’s life where assistance is needed.• Offering and coordinating services and resources for the client.
• Monitoring delivery service and working with providers to ensure person’s needs are met.
• Provide a person-centered service planning with person and creating Coordinated Consumer Support Plans.
• Attend regular meetings with the person and their interdisciplinary teams.
• Travel to different locations in different counties for meetings.
• Case Managers will provide case management for people on EW, AC, Rule 185, CADI, CAC, and IDD Waivers.
• Other duties as assigned.
Case Manager Requirements:
• Applicant is required to be a graduate from an accredited four-year college. Major and/or minor in social work, psychology, sociology or a closely related field is strongly preferred, but not required.• Applicant is required to have a valid driver's license and acceptable driving record as determined by Bestview Care Options policy.
• Applicant must have a reliable vehicle covered by insurance.
• Applicant is required to successfully clear local and/or state background checks.
• Applicant will be required to have internet access at home.
Preferred Qualifications:
• 1-2 years of experience in service coordination or case management is preferred.• Speaking a second language is preferred.
• Social work license preferred.

• Monday to Friday (Schedule will be determined with hiring supervisor)Working Remotely:
• Case Managers are equipped to work remotely at home.
• Case Managers will be required to meet with their supervisor weekly via teleconference or in person.
• Case Managers will be required to attend monthly team meetings in person.
• Case Managers are required to meet with their clients in person.Job type: Full-time (This position is remote, with occasional in person meetings)
