Public Health Nurse (PHN) – Full time
Job Responsibilities:
• Nurse will complete CFSS assessments and submit assessments to the county.
• Nurse will travel to client’s home for face-to-face CFSS assessments.
• Nurse will attend all required county and agency meetings and/or trainings.
• Nurse will consult with waiver/AC program Case Managers on county cases for medications and health changes.
• Other duties as assigned.Public Health Nurse Requirements:
• Applicant must be licensed as a Public Health Nurse and be in good standing with the RN board of MN.
• Applicant is required to have a valid driver's license and acceptable driving record as determined by Bestview Care Options policy. Applicant must have a vehicle that is covered with current insurance. Applicant must have a reliable vehicle.
• Applicant is required to successfully clear local and/or state background checks.
• Applicant will be required to have internet access at home.Benefits:

Waiver Case Manager – Full Time
Job Responsibilities:
• Assessing the areas of the client’s life where assistance is needed.
• Offering and coordinating services and resources for the client.• Monitoring delivery service and working with providers to ensure person’s needs are met.
• Provide a person-centered service planning with person and creating Coordinated Consumer Support Plans.
• Attend regular meetings with the person and their interdisciplinary teams.
• Travel to different locations in different counties for meetings.
• Case Managers will provide case management for people on EW, AC, Rule 185, CADI, CAC, and IDD Waivers.
• Other duties as assigned.
Case Manager Requirements:
• Applicant is required to be a graduate from an accredited four-year college. Major in social work, psychology, sociology or a closely related field is preferred but not required.• Applicant is required to have a valid driver's license and acceptable driving record as determined by Bestview Care Options policy. Applicant must have a vehicle that is covered with current insurance. Applicant must have a reliable vehicle.

Preferred Qualifications:

Job Responsibilities:
• Case Aide will assist Case Managers with case coordination with mailing, services agreements, and office coverage. They will assist with any missing documentation for Case Managers and other staff.
• Case Aide will assist with coordination with counties on forms, reassessments, and communication.• Case Aide will assist with clerical work: Distributing mail items, scanning, emailing, and uploading forms.
• Case Aide will also maintain our hard files by creating and organizing hard files.
• Case Aide will assist with auditing and tracking cases.
• Case Aide will assist with initial contacts for cases by mailing out paperwork, initial phone contact, and coordination with the county on new referrals.
• Case Aide will serve as a backup for other Case Aides and Case Managers.
• Case Aide will be required to be in office as needed for meetings, paperwork, and coverage.
• Other duties as assigned.

• High school diploma or higher.• Must be able to speak and read in Somali.
• Experience in clerical or administrative work.
