  • Public Health Nurse (PHN) – Full time

    Job Description
    • Conduct CFSS assessments for people via phone and/or in their home.
    • Attend all required County and agency meetings and/or trainings.
    • Consult with Waiver/AC program case managers on county cases for medications and health changes.
    • Position requires in home face to face assessments and travel.
    • Additional duties as assigned. Skills/Qualifications/ Required
    • REQUIRED: Licensed as RN. PHN and in good standing with the RN board of MN.
    • Vehicle and car insurance required for travel in county.
    • Must Qualify for employment after criminal background check, per guideline of the MN Department of Human Services Schedule
    • Will vary depending on assessments
    • Hybrid Model- in office and working remotely.

  • Waiver Case Manager – Full Time

    Job Description
    The Case Manager is responsible for:
    • Assessing the areas of the consumer’s life where assistance is needed.
    • Offering and coordinating resources, and services for the person.
    • Monitoring service delivery and working with providers to ensure the consumer’s needs are met.
    • Provide person centered service planning with person and creating Coordinated Consumer Support Plans. • Attend regular meetings with the person and their interdisciplinary teams.
    • Travel to different sites in different counties for meetings.
    • Case managers will be providing case management for people on EW, AC, RULE 185, CADI, CAC, and IDD waivers. • Other duties as assigned. Case Manager Requirements:

  • Case Aide – Karen Speaking – Full Time

    Job Description
    • Case aide will assist case managers with case coordination.
    • Case aide to assist with scheduling appointments for staff, uploading documents into case works, to assist with mail, service agreements, and office coverage.
    • Case Aide will assist with coordination with counties on forms, reassessments, and communication. Database Management: Create new files, placing paperwork inapplicable folders, and maintenance of database information.
    • Case Aide will assist with all mail: Mail Distribution: Distribute mail items, scanning, emailing, and saving in database.
    • Case Aide will assist with auditing and tracking cases.
    • Case Aide will assist with follow-up for missing documentation with providers, PHN/RN and/or case managers.
